to know or not to know
Having 2 of my beloved MakCik to be diagnosed with terminal stage of cancer, makes me realized one or two things...
At this very moment, as I'm writing this one of them is still breathing, Alhamdulillah.. But unfortunately, the other one......... Allah loves her more than we do. Thus, to end all miseries she had to suffer, Allah took her away. Lets pray that she will be together with golongan beriman dan bertaqwa. Amin...
From what Mak told me, MakNgah always complain of having stomachache. She went to see Dr, had a diagnosis of Gastritis, got a list of prescription, and she would buy the same medications whenever the pain comes. This had been years, till finally she realized that her leg is aching. Later it became swollen, and even painful. She needed to recover, as she will be going to Mekah with my late PakNgah. After days of investigations, Dr scanned her leg. To her surprise, there's a tumor there. So, Dr ordered for further investigation of her abdomen. Finally, it was 4th stage stomach cancer, metastasing to bone. As one of my aunt told me, the cancer even spreaded to her lungs. I remembered that day, I was Form 2 back then, Busu came to the hostel, saying," MakNgah just passed away this morning, pack your things."
As the other aunt, she too...Had her diagnosis late, it's already the 4th stage, but she keeps on fighting it. Be strong, dear Aunt!
I dont know about my ancestors. Only 2 of them had/have cancer. It's from Ayah's side. I know, Ayah's side has a long history of diabetes melitus (thus, my great concern on DM), coronay artery disease, hypertension. And not to forget obesity.
Some days ago, my classmates and I, had this discussion about having a screen test, or some sort of investigations. A lot of them chose not to know, to be in the dark. Not knowing is better, as they said.
"Knowing would change you life, instantly'"
Of course, who says it doesnt? What if, your disease is still curable, operable? Surely it will change a thing, right? As for me, I cannot stand to let all the pathological processes to happen inside the body, and doing nothing. And how on Earth I'm going to know if there's a patho process inside? So, the answer is, I opt to go for check ups,and be more concern regarding my health status. Having a relatives with history of late diagnosis of cancer, make me realized, it's worth knowing. And it always be.
The question is, how about if it's already incurable state? Am I supposed to give up? Blame the cancer cells? Blame the genes, for having the predispotion? Blame myself for not going for regular check ups? No, dont blame anything, even yourself. Remember this is just a test. Allah tests you with difficulties, diseases, troubles, sadness. Remember He tests you with victory, joy, wealth, health as well. It's just a test, nothing more. It's a way of Him showing His love. He gives a hint. A hint telling that you will soon meet Him. And are you ready yet? To meet Him, in this case? At least He spares you some time to repent all your sins. To do good deeds, till your last breath. Be happy to get the warning. You know your prognosis, and it is better.
However my friends, even if everything is ok. Be grateful, Allah is kindly lends you more time, age, healthy body. Grab the chance while it lasts, as you never know when He takes it back from you. You are just borrowing it, there is a time limitation. When the time ends, you need to return. Nothing is certain. But one thing for sure, everything must come to end. All living things will die, sooner or later.It just a matter of time. There are preparations to be made. And knowing that you dont know the exact date, time and day; you have to live for the present too. You have to achieve your dreams, goals in life, ambition. Who knows, may be you're going to live for another 50 years or even more?
Having had the thought, I choose to know. Yes, it's worth it!

Ada Apa Dengan Diabetes?
14 Nov. dipilih sebagai hari diabetes sedunia sbb itu merupakan tarikh kelahiran penemu (discoverer) insulin, Federick Banting. Beliau menerima Nobel Prize atas hasil kajian beliau yang gigih itu...Oh ye, untuk yang tak tahu, diabetes atau lebih tepat lagi diabetes melitus tu ialah penyakit kencing manis, bak kata org Melayu.
Kenapa kencing manis bahaya? Secara ringkasnya, paras gula yang tinggi dlm darah bagi tempoh yang lama akan menukar proses metabolik dalam tubuh dan mengubah keadaan salur darah. Keadaan ini menyebabkan berlaku kekacauan dalam semua proses asal. Salur darah menebal, mudah luka dan cepat tersumbat. Proses metabolik lemak terganggu dan akibatnya kandungan lemak pula menjadi tinggi dalam darah. Dan pelbagai lagi kekacauan melanda.
Bagaimana?
Sebenarnya untuk bercerita perihal pencegahan DM dan komplikasi memang panjang. Tetapi hari ini mari bercakap tentang pengetahuan kepada pesakit dan orang ramai sebagai langkah pencegah primer (education of the patient and the public as primary prophylaxis).
Sekitar dewan teater, terdapat banyak pameran berkenaan diabetes,antaranya tentang insulin, glucometer, ubat-ubatan, insulin pen, non caloric sugar dan banyak lagi. Terdapat juga pameran berkenaan penyakit kardiovaskular, alatan dari syarikat Omron, ubat gigi diabetes, jurnal-jurnal perubatan dan lain-lain. Pameran terbuka untuk orang ramai.
Risalah tentang kumpulan baru ubat diabetes: Incretin mimetic. Byetta.
Sampel gula bukan kalorik: Aspartame, jenama Sugar-Free.
Mahu kurus? Terdapat panduan menguruskan badan (lebih kepada promosi ubat sebenarnya) dan perihal Meridia (Sibutramine), sejenis penahan selera (appetite suppressant). Ubat ini membuatkan cepat rasa kenyang di sebalik kuantiti makanan yang kurang.
Sebahagian jurnal perubatan yang ada. Profesor Medvedev (cikgu cardiothoraxic)pun ada published hasil kajian beliau kali ini. Ada juga kajian tentang varicose vein of spermatic cord.
Akhir kata, selamat menyambut hari diabetes sedunia! Cegah sebelum kena.

Pertama-tama kali selamat menyambut Aidilfitri diucapkan kepada semua umat Islam tak kiralah di mana anda berada. Maaf zahir dan batin juga.
Di kesempatan ini, saya nak mengucapkan terima kasih kepada sesiapa yg sudi menjemput ke open house mereka. Terima kasih juga kepada tetamu yg sudi hadir ke jamuan makan-makan kecil-kecilan saya. Maaf kerana saya tak mampu tak undang semua, tiada keupayaan untuk menjamu ramai tetamu. Ribuan maaf kepada tetamu-tetamu saya yang tidak sempat menjamu selera kerana juadah telah habis...Maaf, sebagai tuan rumah saya berasa sungguhlah segan...(-_-)
Walau bagaimanapun, InsyaAllah saya akan mengadakan open house bersama rakan-rakan, maka anda dijemput hadir. Sudi-sudilah, ye.

Be nice, will you?
Today’s entry is not about the first year juniors. It’s for some random juniors. May be you are one of them, or may be not.
As a senior, or precisely a final year student, I don’t mind in helping the juniors. Be it asking some question about academy, asking direction to the hospital, asking masak asam pedas Melaka recipe, asking how to deal with a break up (huh??), asking how to explain Van Der Waals bonding or anything, or even asking me to teach them how to cook bubur lambuk, rendang raya,bubur pulut hitam. You may ask, don’t hesitate…As long as I think that I’m capable of answering you question or helping you out, I’m willingly to do so.
However, be a human being I do have a heart. (Not that heart, anatomically). I do have feelings, you know. Even though, you might be seeing me a ‘cool’, warm-hearted, helpful senior, I might sometimes get hurt. (ouch!)
I know, may be you’re in a hurry. You desperately want to know, for instances how to get to the 4th Maternity House, who is Elena Vladimirovna Smirnova, what busses will go to Cardio Center or any other questions….But please have some manners. Even if you knock my door during wee hours (if I’m STILL awake that time), I will open my door for you. It’s not that I want you to express your gratitude towards me. No, I don’t mind even there’s no thank you word from you. But please, when I say that I forgot the direction, when I say that I forgot the teacher’s name, or I forgot who is the new rector, don’t simply judge me by saying,: “Oh, how come you forgot your teacher?” with a surprising face,plus elevated eyebrows. Or saying something like,” You’re not keeping a habit of remembering/ writing your teacher’s name?” or “Kak Ain, your note is unorganized”.
Dear juniors please understand that sometimes I do forget certain details. Yes, I’m keeping the habit of writing my teacher’s name, but sometimes I forgot on which book I wrote it. You see, I don’t have a note book for a cycle/subject. I have some notebooks, notes on paper which is nicely compiled, photocopied notes and others. Sometimes I write it on the notebook’s cover, sometimes on the paper. Please do not compare me by saying using notebooks for writing notes is better than using papers. Have a mental note that, sometimes my friends borrow my notes on different topics. I find that it is easier to give them the topic that they want, rather than giving the whole book which consisting the topic that I would like to read.
And usually you come asking for notes which I already arranged, labeled and kept them in my cupboard/shelf (I do keep my notes in cupboard). It really took me an effort to find that particular notes for you. Sometimes you return the notes or books just after I arranged my stuff, because of you, I have to rearrange them. But, don’t worry; I’m all OK with that, sharing notes isn’t a problem for me. Even I, borrow some notes from my fellow friends and from the previous seniors. I can understand your need. However, as I said above, please have some courtesies. I can differentiate between a joke and insinuation. I truthfully can deal with jokes, not with the latter.
I should be studying Gynecology for the coming exam. Sorry, I’m totally pissed off with ‘you’(not you, the reader, I don’t think ‘you’ read my blog, but I don’t care). But hey, I still treat ‘you’ nicely. Sorry, I’m a bit sensitive.

Still living..Alhamdulillah
It's been almost 2months since the last update.(ayat2 cliche...). Well, currently Alhamdulillah I'm still alive and breathing the fresh air of kampung. Heh.
There are lots of different living in kampung and kota. In kampung everything move slowly and there's no need to rush. It's like you are blessed with plenty of time, if you're living in kampung. Having had visiting (a.k.a menumpang tidur) at Yanty's house, makes me realize how I'm not ready to live in a hectic place. Waking up at 5.00a.m, going out at 6.30am or else you'll facing a terrible traffic jam, showering with the cold water...I dont think I would manage to do that.*sigh*
I saw school kids walking to the bus station, standing inside the sardine packed bus (but not so packed like in Nizhny), I dont think I can do all that. How I'm so grateful living in boarding school for 5 years, went to primary school that only takes 10mins from home (most of the time Ayah would hantar me.heh). If those kids that I've seen already on the street, walking; while I during that age, still sleeping or just woke up. That makes me think, they are a lot tougher than me, more durable,etc....
Being in a different world, separated from the reality, I feel that I'm not ready for the world. Am I ready for the 'real life'? Am I ready being an adult with huge responsibilities? Am I ready for having a family? Am I ready to be a doctor?
Those questions are difficult to answer, and some are uncertain.
Let's pray I could face the life smoothly,InsyaAllah.
Life, is not always like a bed filled with roses ( without the thorns.heh).Bad things do happen, sometimes.
p/s: congratulations to the seniors on your graduation. Congratulations to dear friends, who are tying the knot, be it a wedding or engagement. My prayer, InsyaAllah will be with you. Semoga berbahagia!

Update
Alhamdulillah, after loosing almost 2days on journeys, waiting for the delayed flights, missing the sunshine; finally we had arrived at Incheon International Airport, Seoul at 9++pm safe and sound. Currently staying with students from Hanyang University (of course with the girls!) in their residential hall. They are really nice, warm-hearted, kind, generous, good people...and the list goes on (if I keep on listing the goodness of them, this entry is totally not enough).
I'm not going to write (read: type) this as though "Dear diary style", may be I'll do something like summary or like some sort of review. (hahaha, perasan journalist ape???)
Ok, today is day-4 in Seoul. So far, I've been to Seoul Grand Park, DongDaeMun, Incheon, Wolmido. I really dunno how to start. I guess all of you can go Wikipedia'ing those stuff to check it out..kahkahkah (main2 puler).
Let's start with Seoul Grand Park....What's the 'bestest' thing I like here is...roses...(kakakaka...jadi minah bunger jap). They planted diff kind and species of roses here. Well, I'm not flower expert, but I do enjoy seing them. There are diff colors, including purple roses. To lazy to read the description of each kind of them, so I cant tell much about them. May be will upload some pictures later. So, Seoul Grand Park consists of The Rose Garden and Zoo Park (didnt go to zoo, zoo for kids..kekekeke). After taking pictures here and there, we headed to Dong DaeMun, to have lunch there. Ikan bakar Korean style..Nothing special about the ikan bakar, but what I'm interested with is the side dishes..There are spicy sweet (fried??) squids, kimchi, some weird green vegies (like sireh or something), spicy bean sprouts and rice (glutinous??? It's kinda sticky-Ok, may be rice isnt the side dish). Among all, I like the squids, even it's liat and made my massetter muscles fatigued, I do love it. I dont mind to eat it all over again (exaggerating, huh?)
Later, the friends took us to ChongGaeChon, it's an artificial river located in the heart of Seoul city (by saying heart, is it in the middle or on the left side like human heart?--- hahaha..lawak medic, sile gelak ye..). Saw many ppl at river bank, I can say that they really enjoyed relaxing there, playing with water, splash here and there.

But, sadly Eileen wasnt there to enjoy the happy moment. May be she was too busy preparing for Infectious D exam. Till then...

Why CheDet?
When people ask me: Who is your hero?
My answer ........was never and will never be Dr. Mahathir Mohammad or also known as CheDet. Having had this man to be my prime minister since the first time I stumbled upon Malaysian politic, CheDet played a significant role in my life during my childhood and as a teenager. Early years knowing him, I never like him. Simply because my father is a hardcore of the opposition. He loved to discuss politic with his fellow friends, and I, as a little kid loved to eavesdrop them.
He, my father would buy magazines and newspapers, of course from the opposition's proxy. However, ayah is not that close minded or 'jumud'. He also buys (till now) newspapers of the mainstream, just to keep the whole family updated with current issues. I remembered enjoying different newspapers for 'my lunch' after coming back from school. My reading 'menu' ranges from Utusan, Berita Harian, Harakah and Utusan Jawi (during the weekends). Yes, ayah bought Utusan and Berita Harian, but later he stopped subscribing for Utusan, as the news more or less were the same for both.
Now, back to CheDet. My father was my great influence for the foundation of my political view. What can I say, I was too opposition when I was a small kid. I talked about politic since I was in Standard Two. I, later realized that it was only me who fond of pembangkang. Ok, nevermind with that.
However, when I'm smart enough to digest whatever I've read, I started to ask ayah: apa salah PM (that time was CheDet), ayah? Ayah would give his answers plus some evidence, facts, hujahan and whatever he thinks it's right. Then, I would go: Owh, Ok...without further argument.
Years passed by, I was in my senior years in high school. Being a headgirl and prefect, I had to go to various leadership courses. Be it Kursus Kepimpinan Negara, Kursus Pemimpin Pelajar, Kursus Patriotisme, I attended it all. I still remember it vividly, it was 2001,when I started to argue once again with ayah.
"Ayah, nape ayah tak suka Dr.M? Apa salah dia?"
Ayah started to tell about Dr.M's corruption, misdeed, injustice in Anwar's scandal, Mega projects etc...
This time, I became argumentative (or was I trying to provoke ayah??). I told ayah that all the projects are meant for us, the rakyat. Please bear in mind, I had this conversation with highly respect towards ayah, plus some good akhlak as a daughter, his my ayah afterall, takkan nak masuk neraka pula.
I dont remember what happened next. All that I remember, ayah said: They brainwashed you 'Ain during the various courses that you've attended. I dont know what poison they used. Now we need to rewash you.
(Ok, I dont remember the exact words, but the idea is something like that)
Since then, I was called 'anak emas Mahathir' by the siblings. I had to tell them, multiple times, that: I'm neither taking the gov's side, nor the opposition's. I'm liberal. Whatever I think is good, then it's good.
However, Najib my kid brother loved to mock me with that gelaran. Najib, it doesnt hurt.
Ok, that is why when CheDet announcing his decision to quit UMNO, I did feel something. It's not that I'm arguing whether his decision is right or wrong. His decision has nothing to do with me. It's just weird I tell you.
Somehow, deep down inside.....CheDet, I feel for you.
p/s: I'm still liberal like I used to be.

Alhamdulillah selesai sudah exam ophthalmology..:)
1) Mentari
Hari ini keluar dari hostel, saya perasan satu benda. Matahari mula bersinar cerah, terang benderang. Rasa macam tidak biasa pula. Selesai exam, ketika tunggu bus pun mentari masih lagi menyinar, panas terik dibuatnya. Ketika itu terdetik pula ttg UV light. Pendedahan mata pada UV light boleh menyebabkan cataract. Detik itu juga saya berasa risau. Saya tidak mahu memiliki kanta semulajadi yang legap sebelum usianya (cataract = opacity of the natural lens)
Semua orang akan menghadapi cataract, sama seperti akan menghadapi maut. Tetapi mendapat cataract pada usia yang muda bukanlah sesuatu yang indah.
Jadi ketika itu saya berkira-kira utk membeli sunglasses (or shades as they call it). Maka, sila jangan salah faham jika selepas ini melihat saya berkaca mata hitam. Sesungguhnya saya bukan lah mahu kelihatan trendy atau hip dan stylo dgn memakai kaca mata hitam. Saya hanya mahu menjaga mata dan terutamanya my natural lens from getting cataract. Harap maklum.
Tetapi ketika duduk di dalam perut bas yang tidak penuh, saya terfikir. Sekarang memang orang perempuan gemar memakai moderate oversized shades, adakah size untuk muka saya yang kecil molek ini? Perlu mencari lagi, perlu memilih lagi. Kesimpulannya jadi malas pula.
2) Bus
I really dont like to ride on bus No.2 (to go to Semashko), simply because it always overpacked and you will stuck in traffic jam at different places:
- at Delyadova Square
- at Opernii Theater
- at Srednoi Rinok
- at Sennaya Square
Here are some alternative routes. And yeah, you have to change buses.
1) Medisinskaya Street - Sovetskaya - Sennaya
Take any buses to Sovetskaya (e.g: 37, 72,85). Stop at Sovetskaya Square and take bus No.16.
By taking this route, you only have to face 2 traffic jam at Sovetskaya Square and at Mikrorayon Vekhnii Pechorii. However, N0. 16 is not really frequent.Time consume for this journey is approximately 30-40minutes.
2) Medisinskaya-Mikrorayon Verkhnii Pechori-Sennaya
This is one of students' fav's alternative.
Take 72, 85 to Mikrorayon Verkhnii Pechori. You may stop here or one stop after. The latter is advisable as you may avoid a traffic jam,plus more buses to offer. Duration of journey: 30-35minutes.
3) Medisinskaya-Gorkii Square(Bolshaya Parovskaya)-Mikrorayon Verkhnii Pechori
In case you have been waiting for No.2, 72,85,37 for almost 10minutes and afraid that you're gonna be the 'teacher's fav again', dont panic and worry.
Take any buses to Gorkii Square, stop at Bolshaya Parovskaya, take No. 58, 40. However time taken for this route might be longer as reach Semashko's Hosp via Cvabodi Square and Minin Square. I never try this before (to get to Semashko's). May be it worth a try.
Anyone with other alternative routes? Feel free to share.
3) Beras dan harga
Beras merupakan makanan ruji di kepulauan nusantara secara amnya, dan rakyat Malaysia secara khususnya.(intro karangan BM). Apa yang perlu kita lakukan terhadap situasi ini? Saya ada beberapa cadangan:
- kurangkan memakan nasi, lebihkan pengambilan protein, dan sayuran. Berita baik untuk pendiet. Memandangkan harga beras yang mahal ini, adalah lebih baik kita kurangkan memakan beras
- Jgn membazir beras dan nasi. Mungkin selepas ini aktiviti membuat bedak sejuk dan tapai akan tergendala dan kemudian terpaksa dihentikan
- Ganti cara pemakanan. Lewat ini ketika tiada kerja, saya akan mengkaji corak pemakanan negara lain di Asia. Mungkin saya akan mengubah menu di atas meja dengan menu dari negara lain (cth: Jepun). Hehe..Tetapi bila saya dah berkeluarga la.
- Ganti beras dengan sumber carbohydrate yang lain. Saya cadangkan kita memakan ubi kentang, ubi kayu atau keledek. Saya prefer ubi kentang. Anda pula?
Cukuplah, 4 cadangan, nanti apabila ada cadangan lagi,InsyaAllah akan saya kemukakan. Jika sesiapa ada cadangan, sudi-sudilah kongsi.
Ok, now I'm tired typing all these. Gtg, wanna bake some cake. Hehehe (^-^)/

Aku tiada schizophrenia,
Tidak perlu melayan hallucination dan delusion,
Tidak perlu ambil anti psychotic yang lebih berkesan jika terkandung lithium.

Aku boleh tgk bunga depan hostel 3 yang mama tanam,
Oh, ya...Jangan pijak ye..
Penat tahu mama tanam,
Mama bilang bunga itu utk kita.
Sekurang-kurangnya,
Even ayam tiada hari ni,
Esok stok baru PPIM sampai,
Walau ayam tiada, masih ada sotong,
Masih boleh menikmati Tomyam Kung....

BTW, balik dari lecture, ku terlihat seorang makcik tua Russian bercakap kepada tong sampah.
Aku pandang sekali, tetapi lama, walau telah melepasi dia, aku masih memandang.
Ku fikir, ade schizo ke apa MakCik ni? Hmmm...sepanjang dlm bus, aku cuba berfikir perihal schizophrenia dan teringat tentang blog seorang pesakit schizo yg aku pernah baca (takde keje ke baca blog, 'Ain???)

Visit Nizhny 2008
In 2006, the service was limited and infrequent. But now, after 2 years, the number of these buses are increasing with more routes to offer. Good thing is students can get monthly bus pass to save their rubles, with the price of 150rub. Perhaps one day, big buses will totally replace the traditional smaller sardin-packed mashrutnoe taksi.
However, taking bus in Nizhny Novgorod require ones to have a special skill. Sometimes, you need to take more than one bus to get to your desired destination, be it Pediatric Hosp, ННИТО (Trauma Hosp), Hosp No. 13 etc.... Sometimes, the bus is REALLY packed, like a tin of San Hose with 4 or 5 sardines in it. You need to cram in, or else will be late to class and being teacher's favourite student (uhuk..uhuk...).
No worries, as an experienced Nizhnian (hehe), here are some guides:
1. Acquaint yourself with more than one bus to the desired destination. Changing 3 buses, sometimes is better and even faster.
Eg: rather than waiting for 68 to Hosp No. 13, try to take any buses to Zhukova Square and take 77 to Hosp No. 13.
2. Use your physical advantage:
a) small (read: cute) person like me: try to 'menyelit-nyelit' between those tall and huge Russians.
b) bigger (tougher??) : try to push the Russians (Russians, not your fellow Malaysians...hehehehe)
3. Be like bird. Early bird catches worm. So, wake up early.

Jgn Pandang Sepi
Hari ini ku membaca akhbar secara atas talian lagi. Tertarik kepada dua buah artikel berasingan. Kedua-duanya mengisahkan kejadian bunuh yang dilakukan oleh pesakit pskiatri. Saya tidak menyalahkan pesakit tersebut. Sesungguhnya mereka tidak dapat berfikir dan merasa seperti kita yang normal. Mereka tiada pertimbangan seperti yang normal, meraka di dunia mereka sendiri. Maka, apa peranan kita dalam membendung permasalahan ini?
Umum akan cuba menjauhi pesakit sebegini. Saya tidak pula menggalakkan anda mendekati mereka sewenang-wenangnya. Anda perlu berpengetahuan dan berhati-hati ketika berhadapan dengan mereka. Namun, mereka masih manusia biasa seperti anda. Mereka lebih tersentuh, hipersensitif, dan adakalanya berada di dunia tersendiri. Tingkah laku mereka di luar jangkaan kita.
Saya tidak mahu berbincang tentang penyakit pskiatri secara satu-persatu. Anda boleh membacanya di dalam buku. Saya ingin mengulas tentang layanan stereotype yang sering mereka terima.
Menggelarkan mereka;'Orang Gila', 'Setengah Suku', 'Tiga Suku' dan lain-lain lagi bukanlah sesuatu yang baik. Malahan boleh membuatkan mereka terasa hati. Mengherdik mereka sesuka hati juga boleh membangkitkan marah mereka. Awas. Ketika mereka marah, tindak balas mereka adakalanya membahayakan. Mengejek, mengeji, mencerca mereka juga boleh menyumbangkan kepada kes-kes bunuh seperti ini.
Sebagai orang dewasa, kita seharusnya menghormati mereka. Mereka tidak hina dan kita tidak pula lebih 'manusia' berbanding mereka. Cuba fahami keadaan mereka. Jika tidak tahu, rajin-rajinlah membaca tentang penyakit mereka. Ajari kanak-kanak kecil untuk tidak bersikap biadap dengan mereka. Pada pandangan peribadi saya, tidak salah untuk anda menasihati anak-anak anda (atau adik-adik) supaya berhati-hati dengan pesakit ini. Namun, amatlah anda perlu menyatakan bahawa mereka sebenarnya sakit, perlu dihormati, jangan diganggu sesuka hati, mudah tersentuh dan lain-lain nasihat yang difikirkan sesuai. Amat penting juga untuk memberitahu bahawa pesakit ini juga manusia sama seperti orang lain.
Kita patut melihat mereka dengan kacamata berbeza kali ini.

Stigmatization
Ini merupakan update ringkas.
Saya sungguh tidak berpuashati dengan generalization terhadap orang Melaka.
Generalization nya berbunyi begini: Orang Melaka bukan kasar ke percakapannya? Suka mencarut-carut?
Persoalan ini dilontarkan oleh En. ClassRep. pada hari ini. Ini bukan yang pertama. Malahan acapkali apabila sesi berbual-bual mengenai negeri kelahiran, soalan ini akan timbul. Kali pertama saya mendengar soalan ini ialah ketika di Mozac Bumi Bertuah. Seorang sahabat berkata: "Orang Melaka suka cakap kasar lah. Dulu aku tidak pernah sebut ba alif ba ya. Sekarang sejak sekolah kat Melaka, aku dah jadi terbiasa."
Saya hairan, saya pelik. Apakah kaitan dengan orang Melaka? Saya tidak pula gemar mencarut-carut. Malahan berkata seperti itu tidak mencerminkan peribadi seorang muslimah dan intelek pada saya.
Perkataan seperti 'lahabau' dikatakan berasal dari Melaka. Namun, saya tidak pernah mendengar orang di kampung saya menyebutnya, samada tua mahupun muda. Atau saya yang kurang bergaul dengan orang2 di kampung? Malahan apakah maksud perkataan itu tiadalah saya tahu.
Apabila perkara ini dibangkitkan, saya akan bertanya kembali kepada si penanya: "Adakah pernah mendengar saya/aku berkata sedemikian? Pernahkah terlihat/terdengar saya berkata kata-kata carut-marut kepada anda/sesiapa sahaja?"
Biasanya, mereka akan terdiam dan membalas seperti:"Kamu/kau lain. Aku dulu ada kawan dari Melaka, mereka begini dan begini".
Saya adalah anak jati Bumi Hang Tuah, ibu bapa saya berasal dari Melaka. Malahan nenek dan datuk saya dari kedua-dua belah ibu dan bapa juga orang Melaka. Saya bersekolah di Melaka sejak sekolah rendah lagi (kecuali darjah Satu. 7tahun pertama saya tinggal di Klang). Saya tinggal di Melaka sejak kecil lagi. Tidak Melaka kah saya? Bukan orang Melaka kah saya? Malahan saya rasa saya sangat 'hardcore' Melaka juga.
Umum hanya melihat perkataan yang tidak elok sahaja. Ada juga perkataan biasa-biasa seperti perkataan 'melesa-lesa'. Melesa-lesa bermaksud sesuatu yang dibiarkan meleret ke lantai/jalan/permukaan bumi dsb; kemudian dibiarkan terus meleret bak menyapu lantai.
Contoh ayat: Cik B memakai kain melesa-lesa hingga hujung kainnya kotor dan sukar dicuci.(mungkin selepas ini saya akan memperkenalkan perkataan biasa-biasa yang selalu disebut oleh orang Melaka)
Jika dilihat situasi sekarang, saya katakan mat rempit (setelah menonton filem Ahmad Idham) dan golongan muda-mudi kotaraya (setelah menonton Haru-Biru) lebih banyak menggunakan kata-kata kesat sebagai basahan di bibir mereka. Maka layakkah saya berkata: "Orang2 kotaraya dan mat rempit gemar berkata-kata kesat"?

Mari Mengundi
Entry kali ini ber'запах' politik.
Isu berbangkit kali ini ialah pemilihan pihak berkuasa tempatan dan JKK setempat.
Saya sangat bersetuju dengan saranan Karpal Singh yang mahu pemilihan PBT yang dimansuhakan pada 1965 diadakan semula. Namun, pemilihan ini mungkin akan berlaku di Pulau Pinang sahaja. Namun, bagi negeri-negeri lain (terutamanya Melaka) masih boleh mencadangkan supaya pemilihan PBT dibuat di negeri masing-masing. Jika perkara ini terlaksana, penduduk setempat boleh memilih datuk bandar, menentukan siapa layak utk menguruskan perihal sampah-sarap, siapa patut gubah bunga di tengah2 roundabout etc... Malahan, dengan pemilihan tersebut, PBT tidak akan buat pekak setiap kali ada aduan, kerana mereka sedar bahawasanya mereka naik pun sebab dipilih orang ramai. Mungkin akan berkurangan kes2 anak tirikan kawasan; contohnya pengurusan sampah di kawasan kediaman datuk bandar dan saudara mara beliau lebih cekap berbanding dengan di kawasan lain. Diharapkan pembaziran yang tidak perlu juga dapat dielakkan. Contohnya, saya sangatlah tidak berpuas hati dengan adanya patung replika gajah di simpang hendak menuju ke Alor Gajah. Saya tahu itu sebagai landmark, tetapi tidakkah itu membazir namanya? Malahan di waktu malam pula, ada lampu lip-lap menghiasi patung tersebut. Bukan itu sahaja, berpuluh-puluh batang pohon kelapa palsu yang dihias cantik dengan lampu kelip-kelip juga 'ditanam' si sepanjang jalan. Tidakkah ini membazir elektrik namanya? Tidak cukupkah dengan meletakkan lampu jalan? Bukankah lebih praktikal? Saya tahu ini cadangan dari kerajaan negeri, tetapi siapa pula yang meluluskan jikalau bukan PBT? Kemudian, dinaikkan pula cukai pintu, tanah, etc bagi menampung pembaziran ini. Jika dilihat masalah jalan berlubang masih tidak diatasi dengan baik, keadaan bus station Alor Gajah masih lagi teruk, dan pelbagai lagi.
Perihal pemilihan JKK pula, yang ini saya sangat2lah bersetuju. Jika di kampung saya, hanya parti tertentu sahaja yang boleh manjadi JKK. Sebabnya? Parti mereka yang menang pilihan raya. Ok, ada logik juga di situ. Tetapi, pada pandangan saya, pemilihan JKK juga sepatutnya lebih neutral dan terbuka. Penduduk kampung patut diberi hak untuk memilih ketua kampung mereka tidak kira parti apa yang mereka undi (sebab undi adalah rahsia). Jika mereka mahu calon kalangan parti tertentu pun, pengundi patut terdiri daripada seluruh penduduk kampung dan bukannya ahli berdaftar parti tertentu sahaja. Jadi masalah lampu jalan rosak dan tidak dibaiki kerana 'kamu orang pembangkang/kamu bukan ahli' sepatutnya tidak timbul. Nasib baiklah si pembangkang itu tadi berjiran dengan Ketua Pemuda parti tertentu itu di kampung tersebut, maka lampu jalan di hadapan rumah beliau cepat dibaiki jika rosak. Bagaimana pula, jika lorong/ jalan tersebut dihuni oleh golongan pembangkang/bukan ahli berdaftar rata-ratanya? Anda boleh melihat perbezaannya.
Saya bukanlah pro mana-mana pihak dan saya bukan anti-kerajaan. Cuma saya tidak bersetuju dengan sistem yang berada di hadapan mata. Saya berasa ianya muak, mual dan meloyakan. Saya mahukan perubahan. Saya benci melihat nepotism berlaku di peringkat akar umbi. Benar, saya tidak mengundi pada pilihan raya yang sudah. Tetapi, saya pasti akan mengundi di pilihan raya yang akan datang. Jika saya boleh mengundi dalam pemilihan datuk bandar, PBT dan JKK sekalipun, saya pasti akan turut serta.
p/s: ini pandangan peribadi penulis. Penulis menggunakan contoh setempat kerana penulis mengetahui perihat tempat tinggalnya. Jadi, pandangan penulis terbatas pada kajian kes setempat dan pengetahuan umun yang penulis tahu. Penulis masih belajar dan cuba meningkatkan pengetahuan semasa lagi. Harap maklum.

Sebuah Cerpen Suspens Tanpa Nama.
Telefon bimbit dicapai, ku hantar pesanan ringkas, seringkas mungkin. Kali ini aku nekad, kali ini aku bulatkan tekad. Aku juga mampu, aku juga kuat. Kuat seperti orang lain.
Sudah berminit2 berlalu. Tiada report bahawa mesejku diterima. Rasa tidak sedap hati pula. Sudah 7.30am, last call! Last call utk bertindak. Ah....aku tidak mahu endah pada amaran itu, last call pun, last call lah. Peduli apa aku.Tapi................aku akhirnya akur. Aku bergegas, pelan ke-2 harus ada, sekurang-kurangnya.
Selesai urusan pertama.Kini aku dipersimpangan delima lagi. Mahu atau tidak? Aku menoleh, berpaling meminta pendapat.
"Camne eh? Nak ke tak?"
Tiada jawapan.
"Nak ke tak?!"
Aku bertegas. Namun tiada jawapan lg.
Ah...gila apa aku agaknya. Mashi Moro ini mana boleh bercakap! Ini objek tanpa nyawa. Aku letak Mashi Moro yang ber'snow cap' biru elok-elok, barangkali dia kesejukan.
Ku kerling jam tangan, sementelahan aku tiada jam dinding. Masih awal lagi untuk bergerak. Aku perlu tetap pendirian, perlu buat keputusan. Aku tidak senang duduk , bak cacing kena antihelmintic preparation rasanya. Aku bangun, cuba untuk membunuh masa. Seterika dicapai, mula menyeterika. Aneh, hari ini rasa mahu menyeterika T-shirt pun ada. Ah, tak perlu, ini aktiviti untuk membuang masa.
Masih banyak masa lg. Aku capai buku. Aku baca lg, baca lg dan lg. Entah apa yang aku baca agaknya.
Time's up!!!! Aku capai beg, pakai jaket hitam, rasa kurang yakin. Keluar tanpa menoleh. Aku berhenti seketika, sebelum pintu dikunci, aku berpesan: "Mashi Moro, jaga bilik elok2. Kakak gi dulu.". Pintu ditutup rapat.
Masuk hari ini, sudah kali ke-3. Sudah kali ketiga aku gagal. Aku gagal ponteng kelas lagi.

Mengapa Harus Menonton?
And Salam Sejahtera to all,
Menonton menjadi aktiviti utk menghabiskan masa lapang bagi sesetengah org. Dan saya yakin, aktiviti ini menjadi pilihan ramai. Jadi, persoalan yang ingin saya bangkitkan ialah: Mengapa Harus Menonton?
Menonton dalam konteks ini merangkumi menonton kaca TV, wayang, cerita bersiri di komputer riba, menonton melalui pancaran LCD dan pelbagai cara-cara lain lagi. Jadi pendek kata, kita berbicara tentang semua yang berkaitan dengan menonton.
Dengan menonton, kita cuba utk berpaling dari realiti. Kita tidak mahu memikirkan masalah yang berkusut di minda. Bak terapi minda, kita seakan-akan berasa lebih tenang, selesa, ceria dan pelbagai perasaan possitif. Tapi malang sekali, efek terapi ini hanya utk tempoh masa yang singkat. Efek selepas (after effect) selalunya tidak lama. Analoginya, seperti mengambil amantadine atau ramontadine utk influenza, therapeutic effect hanyalah tatkala pengambilan, apabila berhenti pengambilan, maka tiadalah kesan antiviral itu tadi.
Selain itu, dengan menonton, kita menjadi org lain. Jika dlm pskiatri, istilah depersonalization digunakan. Namun, penonton ini tadi tidaklah sehingga mengalami gangguan pskiatri, malahan tahap neurosis pun mereka tidak sampai. Ketika menonton, kita akan terbawa dan ter'masuk' ke dalam watak di dalam cerita. Jika perempuan, mahu menjadi seperti heroin. Jika yang llki, mahu menjadi seperti hero, Secara tidak langsung proses 'idolization' berlaku. Jika tidak pun, penonton sekurang-kurangnya berharap dapat berjumpa dengan seseorang seperti hero atau heroin dalam cerita tadi. Tidak percaya? Baiklah, saya ingin bertanya kepada penonton perempuan/wanita, berapa ramai yang selepas menonton cereka bersiri Jepun/Korea, berusaha memuat turun (download) gambar si hero, dan letak sebagai wallpaper? Jika tidak memuat turun gambar pun, anda akan mencari maklumat tentang hero td di majalah atau internet. Apa yang saya cuba sampaikan di sini, menonton menemukan kita dengan sesuatu yang tiada dalam realiti kita.
Saya katakan di sini, bahawa menonton adalah seperti mengambil ganja, opium, dan sebagainya. Menonton menjadikan kita ketagih. Kita sangat selesa ketika menonton dan kita mahukan keselesaan itu lagi dan lagi. Sehinggakan hujung minggu tanpa menonton menjadi semacam tidak kena. Kita mahu selesa, kita mahu lari, kita mahu menjadi orang lain sehingga kita boleh teracun (intoxicated). Utk melawan ketagihan kita kekadang mengalami withdrawal syndrome atau rebound, malahan keadaan menjadi makin teruk. Walaubagaimanapun, period tanpa tontonan tetap berlaku. Period ini berlaku di musim-musim peperiksaan atau ketika workload berlambak.Namun, selepas period ini berakhir, maka bermulalah ketagihan ini sehingga mewujudkan keadaan menonton tanpa henti atau movie marathon. Ia menjadi seperti satu kitaran: menonton-ketagih-tanpa tontonan-makin ketagih-menonton lagi-tambah ketagih-terpaksa berehat menonton-kembali menonton-kembali ketagih .........(kitaran berterusan). Kitaran ini nampak seperti tidak putus pula bak vicious cycle.
Sememangnya tidak salah menonton. Tetapi, berpada-padalah ketika menonton.

Sekatan baru terhadap Iran - Majlis Keselamatan PBB adakan undian hari ini
NEW YORK 29 Feb. – Britain dan Perancis semalam menyatakan keyakinan bahawa Majlis Keselamatan Pertubuhan Bangsa-Bangsa Bersatu (PBB) akan mengadakan pengundian esok dan meluluskan sekatan baru ke atas Iran.
Kedua-dua negara itu menjadi penaja bersama satu resolusi untuk mengenakan sekatan pusingan ketiga ke atas Iran.
Dua sekatan sebelum ini diluluskan secara sebulat suara tetapi sekatan terbaru itu menghadapi bantahan daripada empat anggota bukan tetap yang telah membangkitkan pelbagai persoalan.
Keempat-empat negara itu ialah Libya, Indonesia, Afrika Selatan dan Vietnam.
Duta Britain ke PBB, John Sawers berkata, resolusi tersebut perlu diluluskan jika hendak dibentangkan dalam pengundian esok.
Ia disokong oleh lima anggota tetap pemegang kuasa veto iaitu Amerika Syarikat (AS), Rusia, China, Britain serta Perancis dan dijamin mendapat lebih daripada sembilan undi “sokongan” yang diperlukan dalam Majlis 15 anggota itu sekiranya mahu resolusi tersebut diluluskan.
“Hasrat kami untuk mengadakan pengundian secepat mungkin, dijangka esok, sebaik sahaja resolusi itu disiapkan dalam bentuk akhirnya,” kata Sawers semalam.
Bagaimanapun Duta Libya ke PBB, Giadalla Ettalhi memberitahu wartawan, beliau akan mengundi menentang draf resolusi itu.
Duta Indonesia, Marty Natalegawa juga mengulangi bantahan negaranya semalam.
“Sehingga hari ini, kami merasakan terdapat lebih banyak soalan daripada jawapan.
“Kita berada dalam kitaran sekatan, keingkaran, lebih banyak sekatan, lebih banyak keingkaran, dan di mana kesudahannya bagi Majlis Keselamatan?” soal beliau.
Laporan yang disiarkan pada Jumaat lalu itu menyatakan, kesangsian mengenai kegiatan nuklear Iran pada masa lalu telah diredakan atau dihapuskan.
Perkara itu ditekankan oleh Duta-Duta Libya dan Indonesia.

Jalan-jalan di alam maya, akan terjumpa blog seorang gadis manis yang
menulis dalam entrinya: I hate politics, hate politicians. They all
the same. Kemudian dalam entri lain secara tidak logis dia menulis merungut
darihal petrol makin mahal, barang sudah naik harga atau dalam perjalanan ke
UpTown dia sakit hati membayar tol, dan bla, bla, bla...apa dia ini bengong atau
apa?
Apa dia tidak faham logis atau apa.Politik yang dibencinya, yang tidak
dipedulikannya (justeru dengan bangga dia mengisytiharkan: so that's why i
am not going to cast my vote, i dont even register) adalah satu perbuatan
yang tidak logis dan tidak bersesuaian dengan rungutannya.Bukan, ini bukan entri
untuk berkempen.
Saya seperti sang gadis, i just hate politics (dalam erti kata
kepartian)...tetapi saya pengarang, yakni saya seniman yang mana pengarang
kominis seperti Pramodeya Ananta Toer PUN berpesan 'politik adalah
segala-galanya, politik adalah panglima dalam kehidupan'. Itu kata Pram. Nah,
sebagai muslim, tambah-tambah lagilah saya adalah makhluk politik. Makhluk
politik Islam adalah makhluk yang logis.
Benar, sebagai makhluk politik menyertai parti tertentu belum tentu
menunjukkan kita berpolitik. Tetapi mengambil kira rungutan sang gadis, tentu ia
menjengkelkan kerana dia memang tak mahu membuat pilihan. Kemudian dia memekak
hal yang dia sepatutnya menentukan tetapi tidak dibuatnya.Gadis dengan blognya
itu bagi saya contoh manusia tidak logis. Manusia yang ignorant adalah manusia
yang tidak ada logis. Untuk ignorant, dia juga boleh dihujahkan jahil dan untuk
jahil sekutunya ialah angkuh. Ini gadis yang tidak ada logis, jahil dan
angkuh.Dia pentingkan diri. Kemudian kita rasa meluat kerana dia mengadu-ngadu
dalam blognya (betul, itu blog dia, dia boleh merungut apa sahaja...) yang dia
rimas Malaysia, negara tercintanya jadi sekian sekian dan tidak jadi
sekian-sekian seperti kehendaknya.Lebih meluat, dia ini bukan jenis gadis yang
kita lihat berlegar di Mid Valley atau KLCC tak ada arah. Dia pelajar institut
pengajian tinggi, (pelajar IPT tidak logis? Dalam konteks masyarakat ini kes
mati di pucuk.)

Student's Elbow
Well, minna (Japanese: people)... student's elbow is not just an elbow, literally. It's a diagnosis, a disease. It is also known as olecranon bursitis; an inflammation of bursa (a fluid filled sac or saclike cavity - not that 'bursa' saham) which surrounds olecranon ( a bony projection of ulnar bone at the elbow). Now, do you understand?
How does it happen?
It occurs when a 'VERY HARDWORKING, DOESN'T KNOW WHAT ELSE TO DO EXCEPT STUDYING STUDENT' studying for a long time and resting his/her head in his hand and his/her elbow on the table. Thus, due to prolonged pressure over the olecranon bursa, an inflammation occurs.
Fortunately (or unfortunately), it rarely happens today. May be, it's hard to find a very dedicated student nowadays or studying posture has changed over the past years.
What are other signs?
We may see typical signs for inflammation (swelling, redness, pain, increased temperature, movement restriction) locally on the elbow.
How to treat? (Q for doctors/surgeons)

SAD
Why do we(us) always see most of the russians look depressed during cold season? The answer is (I think), they are actually affected by SAD. They look unhappy, depressed, distressed, impaired socially (true, suke marah2 tak tentu psl dlm bus), no facial expression. But once when warm seoson is approaching, they become friendlier, kind, having good mood and other positive signs. So, by accepting the fact that all the sufferings when dealing with these ppl is something psychological and 'psychiatrical', we may tolerate and understand them more. If they try to yell at you, let it be. If they try to laugh at you, let it be. If they try to hit you, hit 'em back even harder (hahahahaha, apekah???). At the end of the day, you will say this to yourself: "Biar ah..malas nak layan org gile"
Remember, it's psycological...Learn to forgive and forget.
p/s: The author is forced to read some psychiatry stuff for tomorrow class. Forcing someone to read will make that person to loose association with surrounding world; having hallucination, delusion, even aphasia and not to mention agnosia too.

prague
Now I'm in Prague,Czech Rep.!!!! Juz arrived yesterday. Today we will be going for daylight sightseing as yesterday we just strolled down the street at night. Well, my first impression was, Prague looks like other Russian places. But after walking around, seing some places, I think Prague is better than Russia. Most iportantly, a lot of their ppl can speak in English. Their language is just like Ruski Izik except that they use Roman alphabet. Other than that, you can get Bohemian crystals here with cheaper price. From what I've heard, he qualiy is just the same like Swarovski (ni org kedai tu ckp). Hmmm, may be what interest me in Prague is the art and the history behind its ppl. I've been searching some good books to bring back home. Guess what, I found The Satanic Verses, written by Salman Rushdie here. (Tp tak tw lg nak beli ke tak...)

Atrial Septal Defect
Cause is benign. ASD represents ~ 10% of all CHD
Eitiology
· Failure of septal growth
· Excessive reabsorption of tissue
-make ~ 7% of all cases of cong HD
-female:male=2:1
-pulmo vasc resistance is lower than N
-heart failure is rare
-in adulthood significant number of individual who had HF/A.flutter/pulmo vasc D èhad surgery / catheter closure (wh has low risk)wh was performed after 5y.o
3 Types of ASD:
i. Primum
-defect located near the endocardial cushion
-may B part of a complete AV canal defect
-occur in lowermost part of the atrial septum & extend to mitral & tricuspid valve
-usually accomp with AV valve defects
-cleft in ant leaflet of mitral valve
-septal leaflet in tricuspid valve
-in severe case: primum ASD + deformities of mitral & tricuspid valve + VSD = commom AV canal
ii. Secundum
-most common ASD
-hole in region of foramen ovale
-can B isolated or accomp with other cardiac lesion
-hemodynamically significant lesions are accomp with RA & RV dilatation, RV hypertrophy, dilatation of pulmo artery àsignify effect of chronically ↑ volume load on R-side of heart
-s/t accomp with pulmo HPT
-most likely to present in middle to late adulthood, either with ↑ fatigue or with atrial aryhthmias
iii. Sinus venosus
-least common
-assoc with anomalous pulmo venous return to RA or SVC
-located hi in atrial septum
Amount of shunting depends on :
-size of defect
-relative compliance of R+L ventricle
CM:
-infants with ASD rarely present with CHF
-chlidren mostly have no CV symptom
-some pt remain asymp, may B dev easy fatigability as older children or adults
-cyanosis not dev unless pulmo HPT dev
PE:
-periph pulse: normal & equal
-RV impulse at left lower sternal border
-no thrills usually
- S2 at pulmonic area is wide & oftenly fixed
-soft systolic ejection murmur at upper /2nd L sternal border (at RV outflow tract)àdt ↑ flow in pulmonic valve
-No murmur from flow across ASD dt low velocity
Imaging studies:
v ECG: pure RV enlargement
-↑ blood flow via RA, RV, pulmo arteries & lungs
-RV hypertrophy
v CXR:
-cardiomegaly
-RA enlargement
-prominent pulmo artery (↑ pulmo artery vascularity)
Treatment:
1) Secundum ASD:
-surgical / catheterization lab closure (with interventional device)
-perform electively in pt btwn ages 1 & 3 y.o
-mortality rate is <1%
-when closure is performed by age 3y.o, late complication of RV dysF(x) & significant dysrhythmias are avoided
-early : always indicated for infant with CHF or pulmo HPT
2) Non secundum ASD
-prophylactic antibiotic vs subacute bact endocarditis
-significant shunt is still present at 3y.o àclosure is required
3) Primum + sinus venosus à surgical closure
Complication:
a. Pulmo HPT
b. Right to left shunt
c. Infective endocarditis
Patent Ductus Arteriosus:
AD allows blood to flow from pulmo artery to dec part of aorta drg fetal life. Failure of normal closure of DA results in PDA
↓ of pulmo vasc resistance after birth results in L to R shunting of blood & ↑ pulmo blood flow to occur
PDA represent ~ 5%-10% of all CHD
Natural hx of PDA:
-is poorly defined
-3 major deviations:
1. At the same pulmo artery P, the risk of dev of pulmo vasc D > in PDA than in VSD
-in PDA blood go to pulmo artery immediately
2. At same age, risk to egt infective endocarditis > in PDA than VSD coz jet stream induces initial damage
3. The incidence of spont. closure in PDA is v. small. So, dt simple surgery wh is low risk, all single congenital PDA with L to R shunting shud be closed
-some using double umbrella device, some using surgical ligation
Now, w/out surgery, put device intraartery & dt jet stream, the device opened (just like an umbrella)
CM:
Symptoms dpnds on amount of extra blood flow to lung wh dpnds on:
- Size of PDA
- Pulmo vasc resistance
Ø Widened pulse pressure
Ø Continuous machine like murmurà run off of blood from aorta into pulmo circ during diastole (dt P pulmo artery is < P aorta)
-best heard at L intraclavicular area
-palpable thrill
-radiate along pulmo arteries(usually radiate fairly well in ant lung field but poor over post lung field) & well heard over the L back
-begin shortly after S1, rises to a peak at S2, & passes to S2 in diastole where it becomes a decrescendo murmur & fades before S1
Ø Mid-diastolic murmur (often heard at apex) à ↑flow across mitral valve
Ø If the caliber of PDA is wide, > transmission of systemic pressure in pulmo artery occurs. The wider & shorter PDA, the higher RV pressure & the > PV hypertrophy, as well as LV hypertrophy
Imaging Studies:
1) ECG & Echo:
-LV hypertrophy + LA enlargement
-RV hypertrophy (if pulmo HPT is present)
2) CXR:
-full pulmo artery silhouette
-↑pulmo vascularity
Treatment:
· Spontaneous closure is uncommon in full-term infants
· Moderate to large PDA à diuretics + digoxin
· Most PDA can be closed in catheterization lab à coil embolization or PDA closure device (when W =5kg)
· Ico pulmo HPT Ö, operation by age of 1y.o to prevent dev of progressive pulmo vasc D
Ventrical Septal defect
Delaying closure in vent septum btwn 5-6 wks of embryonated life
Variable in size, can B located in any part of vent septum, single or multiple lesion
Location:
1) Subpulmonary (membranous)
2) Perimembranous (most common)
3) AV canal type (give severe defect)
4) Muscular (usually small, may close spont)
Anatomy: 4 components of vent septum:
§ Muscular
§ Membranous (below aortic valve)
§ Supracristal /subarterial(comprises conotruncal tissues)
§ Inlet or posterior (comprises endocardial cushion tissues)
History of VSD
-most common cardiac lesion, making ~30% of congenital HD
-catheterization:- to access
-VSD small enuff to have normal P pulmo artery : P syst flow = 2:1 à no need for surgery if closure x (↑ pulmo vasc resistance xoccur
-small % of baby had HF + admin to hosp à almost always had HF by 2m.o (ico large L-R shunt)
-VSD occur when any of these components fail to dev normally
-most common CH defect à 25% of all CHD
-female = male
-The amount of flow crossing VSD (give significant clinical value) dpnds on:
§ Size of defects
§ Pulmo vasc resistance
§ Variation of both above factors with age
-even large defects are asymptomatic at birth dt hi pulmo vasc resistance. Pulmo vasc resistance usually ↓ over 1st 6-8 wks of life
-at 6-8 wks of life, symptoms of VSD may dev
-often detected at 2-6 wks of life
-murmur at 1st day of life esp ico small or moderate defect
CM:
Often asympt
Clinical course is benign
Normal pulmo artery pressure
§ CHF presenting as fatigue, diaphoresis with feedings + poor growth
§ Pansystolic murmur + thrill à best heard at lower L sternal border (1-2 months, dt vent F 2˚ to large L-R shunting
§ P2 not accentuated
§ Initial:-
-tachyP with ↑ resp effort
-excessive sweating dt ↑ sympathetic tone
-fatigue when feeding (hx of feeding is imp)
-Acyanotic
-Easy fatigability
-CHF in infants only
-hyperactive heart, bivent enlargement
-pansystolic murmur, max at lower L sterna border
P2 accentuated
-Diastolic flow murmur at apex
§ Close spont
§ Prophylactic antibiotic needed to prevent subacute bact endocarditis (if defect xclose)
§ No catheterization
-diuretics
-Digoxin
§ Continued poor growth / pulmo HPT:
-close defect by surgery
-close by devices placed at cardiac catheterization
Surgery & med management of HF
Ico VSD: resp infection, underweight, growth retardation are common
PE:
-L chest deformity
- ↑ precordial activity
- change in ausc sx
Imaging Studies:
1) ECG:
-LA + LV enlargement
-LV hypertrophy
-cardiomegaly
-↑pulmo artery silhoutte
-↑ pulmo blood flow

Adekah setiap kali new year kena smbt? Aku tak penah smbt, nak buat countdown pun tak penah. Satu sbb aku tak buat, dua sbb aku takde perasaan pun, tige nape nak countdown? Tambahan lagi aku mengalami kekecewaan di penghujung tahun dan awalan tahun tempoh hari...Semoga aku sembuh..
