oooo(buat-buat paham :p)
saya pun byk yg tak paham (-_-#)
Hehe best jugak notes tu... But try this nyeh nyeh:)
A 25-year-old man presents with headache, dizziness, and claudication. Blood pressure measurements reveal hypertension in the upper limbs and hypotension in the lower limbs.
Which of the following additional findings would be most likely in this case?
A. Aortic valvular stenosis
B. Notching of inferior margins of ribs
C. Patent ductus arteriosus
D. Pulmonary valvular stenosis
E. Vasculitis involving the aortic arch
Ok, 1st let us see....
This pt is young, having HPT only on upper part of body, hypoperfusion of lower part.
My d(x) is coartation of aorta. To be specific, adult type coartation of aorta.
So, additional sign would be notching of inferior margins of ribs.
The sign dev dt enlargement of collateral vessels to supply the distal part. So, due to this enlargement, erosion had occured and causing notching of ribs.
Dd(x)
Aortic valvular stenosis...patutnye sign of hypoperfusion kat brain(one of the triad symptoms). So, dizzeness takleh la.
Patent ductus arteriosus, only in case of infantile coartation of aorta. Dah nama pun infantile, pt ni umur 25thn. If not mistaken, biasanya infant yg ade cond ni tak hidup that lama.& for adult type, dustus arteriosus must obligatory to be closed.
Pulmo valvular stenosis...nanti ade sign of right heart failure la kan? Peripheral edema, ascites etc..
Vasculitis involving aorta, hmmm minta faiz jawab la..Hehehe
Hehe, btl tak nih? Ni tak tgk buku nih.Klw salah tlg tunjukkan. "If I dont know now,I still can learn."
Hua huah... kak Ain memang sehebat orangnye!~ Betul!!!~
